CAMPUS CLEARANCE FORM

Campus Clearance Form

Campus staff use this form to provide provisional enrollment for students who do not have all of the necessary documents to meet residency requirements at the time of enrollment due to:

1. Sharing a Residence with another family by mutual agreement, but do not meet McKinney-Vento definition as homeless.

2. Recently moved - parent can provide lease/rental agreement, mortgage, homestead documents, but is waiting to receive a new bill or govt. document due to the recent move. Note: If all bills are included in the rental agreement, highlight that on the lease - then request car registration, or other govt. documents to fully clear enrollment within 30 days.

3. Missing other documentation as applicable.
